The Lincoln Parish Community Network will hold an Application Roundup for Preschool Aged Children on Thursday April 8 from 8 am – 1 pm and 5-7 pm at the Lincoln Parish Library Events Center.
A child’s official guardian (I.D. Required) MUST BRING the following mandatory documents when applying:
Age: Pre-Kindergarten students MUST be four on or before September 30, 2021
Original Birth Certificate with Official State Seal (Guardian indicated must sign paperwork)
Student’s Official Social Security Card
Current Immunization Record (Must be up-to-date)
Proof of Residence: Two current items of verification (utility bill, cable bill, telephone bell)
***Not required for childcare sites***
Income verification required for all applications (2 recent consecutive pay stubs, W2, SNAP letter, or letter from employer)
Applications received AFTER April 8 may go on the waiting list. Childcare centers accept applications year round. After April 8, contact sites for information about availability. Baseline screening required after placement.
